RESUMO

The study investigates the influence of Voluven 6% and HAES-steril 10% on the hemodynamics and organism water balance of 40 children from 3 months to 17 years of age, which were divided into two groups according to the type of the administered colloid. It is acquired that infusion of colloids with 1:3 ratio compared to crystalloids in general volume of infused liquids (Voluven 6% in the dose of 5 ml/kg/hour in case of median blood loss of 15% of the total circulating blood volume during two hour long surgery and HAES-steril 10% in the dose of 4 ml/kg/hour in case of the blood loss up to 25% of TCBV) allows to effectively neutralize hemodynamic changes based upon administration of anesthetic agents and intraoperative fluid loss. While administration of Voluven 6% is accompanied by significant, statistically accurate decrease of lower limb impedance, which indicates the increased amount of water in them, HAES-steril 10% administration leads to redistribution of water in the body segments with its predominant significant increase in the torso.

RESUMO

The gastroscopic examination has been done in 72 children for diagnosis of congenital pylorostenosis in 1993-1995. The authors think that this method is more reliable and sparing to compare with any others for diagnosis of congenital pylorostenosis. The absence of pylorus motility as a reaction for the external stimulation is a main criterium for endoscopic diagnosis of congenital pylorostenosis.

Changes in vital functions in endotoxicosis were appraised in 41 patients with acute peritonitis during stages of intensive therapy in a complex fashion. Correlative relations were revealed between the severity of endotoxicosis and the function of blood circulation, blood gases, and respiration. Endotoxicosis was characterized by increased level of middle mass molecules, circulating immune complexes, leukocytic index of intoxication, asparagate and alanine transferase. Hemodynamics functioned in hyperdynamic regimen in the reactive stage of peritonitis, in the normodynamic regimen in the transitional stage, and in the hypodynamic regimen in the toxic stage. The immune status was inhibited in the toxic stage of peritonitis. Normalization of vital functions in the postoperative period occurred in parallel with decrease of the severity of endotoxicosis. The studied values approximated the average-age indices in patients with the toxic stage of peritonitis by postoperative days 7-10; sorption methods of detoxification were included in the complex intensive therapy measures in these children.

RESUMO

On the basis of the experience with the use of different methods of intestinal decompression in 56 children with severe peritonitis of appendicular genesis, the authors came to conclusion that intubation of the small intestine via a cecostomy is safe, technically simple, convenient for the care, provides reliable intestinal decompression, do not require the reoperation.
